Scoliosis Screening
Odyssey Charter School will conduct a Scoliosis Screening Program to find middle school students (6th and 8th grades) who may have curvature of the spine. The screening will be held during the day in the school clinic.
If you do not wish for your child to be screened at this time, please fill out the form and return it to the school clinic by Friday, October 13, 2023. If the form below is not returned, your child will be screened with other students in their grade level. Screenings will be conducted beginning Tuesday, October 17, 2023.
